2026 PVL Fee Adjustment Application Form

This form is for PVL players who would like to have PVL fee adjusted due to injuries and were absence for a large number of PVL games. If you only missed a few games or were away due to holidays or work, fee adjustment won't be considered. Applications will be considered case by case and there is no guarantee it will be approved. Please provide as much detail as possible to support your application, including medical certificate etc. You will be advised if your PVL fees have been adjusted.
