  • Purpose: This questionnaire helps our church understand you, encourage your walk with Jesus Christ, and develop a ministry that meets spiritual, personal, relational, educational, professional, and mission needs.

    You do not have to answer every question. Please answer honestly. There are no right or wrong answers.

    • DISCOVER PURPOSE — Understand God's calling and discover spiritual gifts.
    • SERVE OTHERS — Use their gifts to serve the church and community.
    • LIVE ON MISSION — Share the Gospel and demonstrate God's love wherever God has placed them.
