The Medicare GLP-1 Bridge program is NOT available to patients with a diagnosis that otherwise provides a pathway to GLP-1 coverage through their Part D drug plan. Please tell us if you have been diagnosed with any of the following conditions.
Have you been diagnosed with type 2 diabetes? The ADA defines diabetes with an A1c > 6.5% or fasting glucose > 125 mg/dL.
Have you been diagnosed with moderate or severe obstructive sleep apnea? This is usually diagnosed with a sleep study and treated with a CPAP or BiBAP machine.
Was your obstructive sleep apnea diagnosed with a sleep study?
What was your Apnea-Hypopnea Index (AHI) from your sleep study?
Have you been diagnosed with MASH (fatty liver disease with scarring)? This is usually diagnosed with a liver biopsy or FibroScan specialized ultrasound.
