Program Consent and SLC Expectations

You’re almost there! The last thing we need are a few forms signed. For this section, grab your parent/guardian, and complete the following forms:
Informed Consent and Acknowledgement of Risks - this form is mandatory and gives permission for your participation in the program.
Muskoka Woods Waiver - this form is mandatory for participation in the in-person gathering.
