Complete one form for each child being registered. You will receive confirmation of your registration and schedule to the email provided in the form (on occasion, this email can end up in your spam folder depending on your email preferences).
After submitting registration, complete participation forms for each child registered. (Participation forms only need to be completed for students one time per year and not each time they are registered.) Participation forms must be completed before students participate in programming.
Important Information:
Programming will be held at Louisville Elementary School, Exterior Room Door 18. (Note: Lousiville Elementary School students attending sloyd at the 3:00PM session may join the classrom directly from the interior of the school. All other students, please enter through the exterior classroom door and not the main school entrance. Directions to the classroom location will be included as an attachment in your registration confirmation email.)

Pricing Overview (per child):

An invoice will be completed and sent to you separately from this registration form, and you will be able to pay electronically or by cash/check. Invoices are due upon receipt.
