  • **Reminders on Voucher Approval**

    • Vouchers must be received by 4:00 p.m. on Wednesday to be considered for payment on Friday.
    • Your attached Excel/PDF timesheet must state each charge Case #, $ Amount per charge, and $ Amount total.
    • Each reported $ Amount per charge and $ Amount total must match between this form and the attached timesheet. 
    • A flat rate can be used once per client caseload, not per client's charge.
    • Flat rates do not require an attached timesheet, but you must include a (0.1 x $ hourly rate) fee for each subsequent charge, and include all charges in this voucher form.
      • E.g. $400 flat rate on case 1, $11.50 on case 2, total voucher request $411.50
    • Cases with a disposition date older than 90 days may be rejected for payment.  
    • If you use the new rates on a case opened after 5/1/23 but the client has pending charges prior to 5/1/23, you may only bill the new rates on the new cases received after 5/1/23.

    The form is interactive and will calculate the running total of all your cases and generate a total on the last page.

  • LPDO is continually working to improve our data metrics without overburdening our attorneys. This year, there are two new fields we ask you to fill:

    • Top Charge Case Category
      • Beginning Oct 1, 2026, this new field will be included on every new Notice of Assignment. It is an internal metric based on case complexity that will help us monitor caseload burden amongst the panel. 
      • For assignments prior to 10/01/2026: Leave selection blank on voucher form.
      • For assignments following 10/01/2026: Refer to the client's Notice of Assignment sheet. If your client has multiple NOA’s, refer to the most recent NOA. 

     

    • Total Billable Hours
      • Use the total billable hours from your timesheet sum total
      • Total Billable Hours = total hours worked + 0.1 entry for each additional charge
      • This value must match what's reported on your timesheet

  • Billables Information

    Flat rate vouchers do not require a time sheet. Hourly vouchers will not be accepted without a time sheet.  Thank you for all your hard work and for serving on the LPDO panel!
