The undersigned hereby acknowledges receipt of a company-owned or leased motor vehicle. I understand this vehicle is being provided for business purposes only. Any use other than to further the business interests of Warren Well Service is not approved. I further understand that I do not have permission to use the vehicle in violation of any of the prohibitions and safety measures below.
I understand this vehicle must be regularly maintained and serviced according to the service schedule outlined in the Owner's Manual or the instructions issued by Coltson Warren, whichever is appropriate.
Further, it is agreed that this vehicle will be operated safely. This includes following company safe driving practices and policies, customer and private landowner expectations, applicable local and state laws and regulatory requirements. I agree to follow the Warren Well Service driver distraction policy. I agree to wear my seat belt whenever the vehicle is in motion and require all other occupants to do the same. I agree to ensure all passengers understand and comply with their responsibility to remain awake, alert, and not distracted to help the driver do the same.
I will not operate this or any other company motor vehicle while under any influence of alcohol or drugs (legal or illegal). Alcohol or illegal drugs are never allowed in a company vehicle. I will not operate this or any other company motor vehicle when impaired by a health, physical, or mental condition, or fatigued.
I understand firearms are not allowed on company or customer property. I agree firearms will not be stored or transported in a company vehicle or personal vehicle being used for company business. I agree to promptly report all incidents. Incidents resulting in any injury or damage to any vehicle or other property, no matter how slight, should be reported to Coltson Warren as soon as it is safe and appropriate.
I understand the articles of this agreement apply regardless of who is operating this vehicle. Only company employees pre-approved by Warren Well Service may operate a company-owned or leased motor vehicle. I understand that no unauthorized personnel may ride in company vehicles. This includes hitchhikers, spouses, family, and friends.
I understand that I am responsible for the security of the company vehicle assigned to me. I understand that I must shut off the vehicle engine, remove the ignition keys, and lock the vehicle doors whenever I leave it unattended.
I understand that company vehicles may be monitored for certain information, including but not limited to geographical location, excessive speeding, excessive accelerations, harsh braking, and seat belt violations. I understand that my driving behavior is part of my overall performance evaluation and could lead to disciplinary action up to and including termination of employment.
I will be financially responsible for all traffic and parking violations that occur while the vehicle is assigned to me. I will also be financially responsible for any injuries or property damage that occurs during non-permissive use.
I understand I must maintain a United States-issued driver's license valid for the vehicle being operated. Further, I herewith grant Warren Well Service the right to investigate my motor vehicle driving record and criminal background at any time for all states and counties where I have resided for the past 5 years.
I agree to inform Coltson Warren immediately of any moving motor vehicle violations or accidents that occur while driving any motor vehicle for company or personal purposes.
I understand I am not to modify the vehicle in any way without written permission. This includes but is not necessarily limited to: safety equipment, settings and features, trailer hitches, stereo equipment, window tinting, navigation systems, security systems, accessibility aids, cargo containment means or racks, material handling means, mobile telephone equipment, changing tire or rim size, changing body configuration, and increasing the carrying/hauling capacity of the vehicle or trailer.
Further, I will not take this vehicle out of the approved work area or United States without written permission from Coltson Warren. I understand that operating this vehicle safely is my responsibility. If this vehicle becomes unsafe, I am responsible for stopping operation and notifying Coltson Warren immediately.
If at any time I fail to meet my company's approved driver criteria, I acknowledge that disciplinary action may be taken, including driver training, loss of driving privileges, and up to termination of employment.
