I confirm that the above entered athlete is eligible to compete for Cornwall & that they have not competed for another County team/County Championships (other than School County teams) in the calendar year 2026 and that they have a valid URN (not for U10’s). Athlete URN is the number given to all competing athletes when registered with England Athletics - This is a mandatory requirement
If a parent/guardian of an athlete under 18, you give permission for the athlete to compete in the Cornwall County AA Cross Country Championships, knowing that this will be run on undulating and varying surfaces which could result in injury.
All details must be shown including full address, date of birth, and stating whether claiming eligibility by birth, 9 months residence or forces
