Q1. Tell us about the member/individual being nominated.

• What relevant positions, roles or responsibilities do they hold?

• What were their results at major competitions (e.g. state, national, international)?
• For athlete awards, have they been part of a representative team and if so, how did they contribute to the success of that team?

Q2.  How are they a positive role model, a good sportsperson and/or an ambassador for Surf Life Saving?

• How do they contribute to broader lifesaving activities and engage with other members (including juniors)?

Provide an overview of the incident, considering some of the following: 

a) Skills and Resources Application 

• Did the rescue / incident require advanced technical skills?
• Was there optimal use of skills within the available personnel? 

b) Environmental Conditions

• Was the incident around rocks, or other precarious situations? 
• What was the size of the surf, temperature, time, visibility, weather, and other environmental conditions at land or sea? 

c) Potential Outcome

• Would the outcome have been the same if the rescuers hadn't intervened?

Q2.  How has the initiative engaged and benefited participants and communities, and responded to their needs throughout its development and implementation?
