      1. Warrants the accuracy and authenticity of the information provided in the Reward Circle Membership Application Form and the identification documents submitted to Okada Manila, and agrees, authorizes, and expressly provides consent to Okada Manila to do any of the following:
        1. To validate the information and government-issued photo identification documents submitted by the applicant with the relevant government agencies; and
        2. To conduct background checks through third-party service providers duly engaged by
          Okada Manila, including but not limited to credit checks, bankruptcy checks, and other
          relevant due diligence verifications as may be necessary for compliance purposes.
      2. Declares that the foregoing information is true and complete and that the Applicant has read and accepted the Terms and Conditions of the Okada Manila Reward Circle Program.
      3. Voluntarily and expressly consents to Okada Manila processing, collecting, using, retaining, storing, and, when necessary, sharing the Applicant’s Personal Data for legitimate purposes as provided in Okada Manila’s Privacy Policy at https://www.okadamanila.com/privacypolicy.
        The Applicant’s Personal Data may be processed for a period of not more than five (5) years
        from the date such Personal Data is collected or until the expiration of the retention period under Okada Manila’s policies.
      4. Further acknowledges that the Applicant may access, update, or correct the Applicant’s Personal Data, or withdraw consent to the processing of any such information at any time by communicating with Okada Manila’s Data Privacy Officer at DataPrivacyOfficer@okadamanila.com.
