By signing below, I acknowledge that participation in the AIMHO Golf Tournament involves inherent risks, including injuries resulting from golf activities, weather conditions, golf cart operation, and the actions of other participants. I voluntarily assume all risks associated with my participation. I release and hold harmless AIMHO, tournament organizers, sponsors, volunteers, golf course personnel, and affiliated organizations from claims or liabilities arising from participation in this event, except where prohibited by law. I certify that I am physically capable of participating and agree to abide by all tournament and golf course rules. I grant permission for photographs or videos taken during the event to be used for organizational and promotional purposes without compensation. I have read and
voluntarily agree to these terms.
