I am the parent or legal guardian of the minor named above. I give my permission for my child to participate in volunteer activities with First Presbyterian Church of Hayward (hereinafter "FirstPres").

I understand that volunteer activities may include community outreach, service projects, food distribution, events, administrative activities, facility projects, landscaping/garden projects, and other activities approved or supervised by FirstPres. I understand that participation is voluntary and that my child is expected to follow the instructions, safety procedures, and policies of FirstPres.

I understand and acknowledge that I must remain present with and actively supervise my child for the entire duration of the volunteer activity. If I am unable to remain present and actively supervise my child, I understand that my child may not participate in the volunteer activity and may be required to leave the volunteer site with me. I agree to:

• Remain on FirstPres's premises and in the designated volunteer area for the entire time my child is volunteering;
• Actively and attentively supervise my child throughout the volunteer activity;
• Ensure that my child follows the FirstPres policies, safety procedures, and reasonable instructions;
• Be responsible for my child's conduct, behavior, and safety;
• Remain available to respond to any questions, concerns, injuries, or other needs involving my child;
• Immediately notify FirstPres staff if I need to leave the volunteer activity or if my child can no longer participate safely.

I understand that FirstPres staff and volunteers are not responsible for providing individual supervision of my child and that my presence does not transfer responsibility for my child's supervision to FirstPres.

I acknowledge that I am responsible for knowing and communicating my child's relevant medical information. I agree to inform FirstPres, as appropriate, of any medical condition, allergy, medication, physical limitation, or other circumstance that may affect my child's ability to safely participate. I understand that I am responsible for having access to my child's medical information during the volunteer activity and for communicating relevant information to emergency responders or FirstPres staff if necessary.

I understand that I am responsible for providing accurate and current emergency contact information for my child. Because I am required to remain present and supervise my child throughout the volunteer activity, I understand that I am the primary emergency contact for my child while participating.

I agree to notify the Host Organization of any changes to this information before my child participates.

I understand that volunteer activities may involve ordinary risks, including but not limited to:

• Slips, trips, falls, and accidental injuries;
• Lifting, carrying, moving, or handling materials or equipment;
• Outdoor activities and exposure to weather or environmental conditions;
• Interaction with members of the public, guests, clients, or other participants;
• Exposure to communicable illnesses or infectious diseases;
• Use of tools, equipment, or other property;
• Transportation to or from volunteer activities; and
• Other risks associated with the particular volunteer activity.

I understand that not all risks can be anticipated or eliminated.

I understand and acknowledge the ordinary risks associated with my child's participation and voluntarily authorize my child to participate.

I agree to exercise reasonable care in supervising my child and to follow all applicable FirstPres safety procedures and instructions.

I, on behalf of myself and my child, assume all risk to my child or property arising from my volunteer service, including, but not limited to, injury and/or death. I release and discharge FirstPres, its officers, directors, employees, agents, and volunteers (Releasees) from all claims, demands, and actions for any damage and/or injuries sustained to my person and/or property in conjunction with my volunteer service to FirstPres. I also agree to hold FirstPres and its Releasees harmless from any claims, demands, and actions arising from my volunteer service. I also waive the protection afforded by California Civil Code Section 1542, which provides:

A general release does not extend to claims which the creditor [i.e., volunteer] does not know or suspect to exist in his or her favor at the time of executing the release, which if known by him or her must have materially affected his or her settlement with the debtor [i.e. FirstPres].

I understand that I am expected to remain present and available throughout my child's volunteer activity. If my child experiences an emergency, I authorize FirstPres and its designated representatives to contact emergency medical services or take other reasonable emergency measures when circumstances require immediate action. I understand that emergency responders may provide medical care according to their professional judgment and applicable law. I understand that FirstPres is not responsible for the costs of medical treatment, emergency transportation, or other medical expenses incurred by my child, except as otherwise required by law.

I understand that my child is expected to follow reasonable instructions from FirstPres staff and to comply with applicable safety and conduct policies.

As my child's supervising parent/guardian, I agree to address unsafe or inappropriate behavior and to ensure that my child participates in a manner that respects staff, volunteers, guests, clients, and other community members. I understand that FirstPres may ask my child to discontinue participation if their conduct or participation creates a safety concern or is inconsistent with FirstPres policies.

I understand that my child and I may encounter private or confidential information while volunteering. I agree not to disclose, photograph, record, or share private or confidential information concerning individuals served by FirstPres.

I agree to ensure that my child also follows applicable confidentiality, privacy, photography, recording, and social media requirements.

I understand that my child is responsible for personal belongings brought to the volunteer activity. I agree that FirstPres is not responsible for loss, theft, or damage to personal property, except as otherwise required by law.

By signing below, I acknowledge that I have read and understand this Youth/Minor Volunteer Liability Waiver.
