I am the parent or legal guardian of the minor named above. I give my permission for my child to participate in the volunteer activity identified above, which is being hosted by the Host Organization (First Presbyterian Church of Hayward, hereinafter "FirstPres") and organized or coordinated by the Sponsoring Organization/Group.

I understand that my child will participate as part of the Sponsoring Organization/Group and that the Sponsoring Organization/Group will provide adult chaperones to supervise participating youth.

I understand and acknowledge that the Sponsoring Organization/Group is responsible for the supervision and care of my child while participating in the volunteer activity.

I understand that the Sponsoring Organization/Group is responsible for:

• Providing appropriate adult chaperones to supervise participating youth;
• Knowing which youth are participating in the volunteer activity;
• Maintaining accurate emergency contact information for each participating youth;
• Obtaining and maintaining relevant medical information for each participating youth;
• Knowing about any allergies, medications, medical conditions, physical limitations, or other circumstances that may affect a youth's safe participation;

• Ensuring that appropriate adult chaperones have access to necessary emergency and medical information;
• Communicating relevant medical or emergency information to emergency responders or Host Organization staff when reasonably necessary;
• Responding to medical, behavioral, safety, or other concerns involving the youth participants in its care;
• Contacting the appropriate parent, guardian, or emergency contact when necessary.

I understand that FirstPres is not responsible for collecting, maintaining, or knowing the individual medical or emergency contact information of youth participants who attend as part of a Sponsoring Organization/Group.

I understand that participation in volunteer activities may involve ordinary risks, including but not limited to:

• Slips, trips, falls, and other accidental injuries;
• Lifting, carrying, moving, or handling materials or equipment;
• Outdoor activities and exposure to weather or environmental conditions;
• Interaction with members of the public, guests, clients, children, or other participants;
• Exposure to communicable illnesses or infectious diseases;
• Use of tools, equipment, or other property;
• Transportation to or from volunteer activities;
• Risks associated with emergency situations or unforeseen circumstances; and
• Other ordinary risks associated with the particular volunteer activity.

I understand that not all risks can be anticipated or eliminated.

I understand and acknowledge the ordinary risks associated with my child's participation and voluntarily authorize my child to participate in the volunteer activity.

I agree that my child will follow reasonable safety instructions and the rules and policies communicated by FirstPres and Sponsoring Organization/Group, and will make every effort to notify a chaperone of the Sponsoring Organization/Group if an issue arises.

I, on behalf of myself and my child, assume all risk to my child or property arising from my volunteer service, including, but not limited to, injury and/or death. I release and discharge FirstPres, its officers, directors, employees, agents, and volunteers (Releasees) from all claims, demands, and actions for any damage and/or injuries sustained to my person and/or property in conjunction with my volunteer service to FirstPres. I also agree to hold FirstPres and its Releasees harmless from any claims, demands, and actions arising from my volunteer service. I also waive the protection afforded by California Civil Code Section 1542, which provides:

A general release does not extend to claims which the creditor [i.e., volunteer] does not know or suspect to exist in his or her favor at the time of executing the release, which if known by him or her must have materially affected his or her settlement with the debtor [i.e., FirstPres].

I understand that my child will be supervised by adult chaperones provided by the Sponsoring Organization/Group.

I acknowledge that FirstPres staff and volunteers are not serving as my child's individual supervisors or chaperones and are not responsible for providing supervision in place of the Sponsoring Organization/Group.

The Sponsoring Organization/Group is responsible for maintaining an appropriate adult-to-youth ratio and ensuring that youth participants remain appropriately supervised throughout the volunteer activity.

I understand that my child is expected to follow the reasonable instructions of FirstPres staff and Sponsoring Organization/Group chaperones.

I understand that FirstPres may ask my child or the Sponsoring Organization/Group to discontinue participation if conduct or circumstances create a safety concern or is inconsistent with FirstPres policies.

I understand that my child is responsible for personal belongings brought to volunteer activities. To the fullest extent permitted by law, I agree that FirstPres is not responsible for loss, theft, or damage to personal property brought to or used during volunteer activities, except as otherwise required by law.

I understand that my child may encounter private or confidential information concerning individuals served by FirstPres. I agree to instruct my child not to disclose, photograph, record, or otherwise share private or confidential information encountered through volunteer service. My child agrees to follow FirstPres's applicable confidentiality, privacy, technology, and social media policies.

I understand that my child's participation is voluntary and does not create an employment relationship with FirstPres. I understand that my child will not receive wages, salary, employee benefits, or other compensation for volunteer service, except where otherwise required by law.

I have read and understand this Youth/Minor Volunteer Liability Waiver and understand the roles and responsibilities of FirstPres and Sponsoring Organization/Group.
