• Service Agreement, Consent & Policies

  • This agreement explains the services provided by The Social Age, the responsibilities of the client and/or authorized representative, payment and cancellation expectations, emergency procedures, and important limitations of service.

     

  • NATURE OF SERVICES

  • The Social Age provides non-medical companion, engagement, activity, personal support, and permitted CNA-level assistance based on the client's individual needs and agreed-upon service plan.

    Services may include, as applicable:

    • Companionship and socialization
    • Recreational and meaningful activities
    • Dementia-friendly engagement
    • Reading, games, crafts, music, baking, hobbies, and current events
    • Routine check-ins
    • Meal preparation and mealtime companionship
    • Light household tasks related to the client's immediate needs
    • Personal care assistance within the permitted scope of the individual providing the service
    • Dressing, grooming, toileting, mobility, and other appropriate activities of daily living
    • Routine reminders
    • Respite companionship
    • Pet-related assistance when agreed upon
    • Birthday and special occasion visits
    • Other mutually agreed non-medical services

    Services are individualized and may vary based on the client's needs, safety, staff qualifications, service duration, and agreed service plan.

  • NON-MEDICAL SERVICE NOTICE

  • The Social Age is a non-medical service provider.

    The Social Age does not replace a physician, nurse, home health agency, emergency medical service, or other licensed healthcare provider.

    Unless specifically permitted by law, within the qualifications of the person providing care, and expressly agreed to by The Social Age, services do not include nursing assessment, medical diagnosis, medical treatment, sterile procedures, medication administration, or other services requiring a healthcare license beyond the worker's credentials and authorized scope.

    If a requested task cannot safely or legally be performed by The Social Age, the client or representative will be informed.

  • SERVICE CONSENT

  • I voluntarily request and consent to services from The Social Age.

    I understand that services will be based on information supplied by the client, family, authorized representative, and/or other permitted sources.

    I agree to provide accurate information regarding conditions, mobility limitations, behavioral concerns, allergies, safety risks, equipment, and other circumstances that could reasonably affect the client's services or safety.

    I understand that The Social Age may modify, pause, or decline a task or service if staff reasonably believe it is unsafe, outside the agreed scope of services, outside their training or credentials, or prohibited by law.

  • EMERGENCY & SAFETY AUTHORIZATION

  • The Social Age is not an emergency response service and does not provide continuous medical monitoring unless a specific continuous service period has been scheduled.

    If a team member reasonably believes the client is experiencing a medical emergency or immediate threat to health or safety, I authorize The Social Age to:

    • Call 911 or other appropriate emergency services
    • Provide information reasonably necessary to emergency responders
    • Contact the client's designated emergency contact(s)
    • Remain with the client when reasonably possible until emergency responders or an appropriate responsible person arrives

    I understand that The Social Age cannot guarantee emergency outcomes and is not responsible for decisions made by emergency medical personnel.

    Any ambulance, hospital, emergency treatment, or other third-party expense remains the responsibility of the client or responsible party.

    Home Safety
    The client/family agrees to maintain a reasonably safe environment for services, including appropriate access, functioning utilities, safe walkways, secure animals when necessary, and disclosure of known hazards.

    The Social Age may stop or postpone a visit if conditions present an unreasonable risk to the client or team member.

  • CHANGES IN CONDITION

  • The client or representative agrees to notify The Social Age of significant changes that may affect services, including changes in:

    • Mobility or transfer ability
    • Fall risk
    • Cognition or behavior
    • Hospitalization or recent medical events
    • Infectious illness
    • Equipment needs
    • Living arrangements
    • Emergency contacts
    • Other safety-related circumstances

    The Social Age may recommend reassessment of services when needs materially change

  • SCHEDULING

    Requested dates and times are subject to availability.The Social Age will make reasonable efforts to provide scheduled services but cannot guarantee that a particular team member will always be available.The client will be notified as soon as reasonably possible of significant scheduling changes.
  • PAYMENT POLICY

  • The Social Age uses Setmore for appointment scheduling and payment processing. Payment requirements and the total cost of the selected service will be displayed during the booking process.

    Payment & Booking
    Payment is required at the time of booking unless another payment arrangement has been approved by The Social Age in advance. An appointment is not considered confirmed until the required payment has been successfully completed and booking confirmation has been received.

    Clients should not submit credit or debit card information through The Social Age's website contact, interest, intake, or consent forms. Payment information should only be entered through the designated secure payment process provided during booking.

    Additional Time or Services
    The scheduled visit includes only the service duration and services booked. If additional time or services are requested, they are subject to team member availability and may result in an additional charge.

    The Social Age will not intentionally extend a visit for an additional charge without the client's or authorized representative's approval, except when remaining temporarily is reasonably necessary because of an immediate safety concern or emergency.

    Pricing
    Current pricing is displayed when booking and/or on The Social Age's website. The price confirmed at booking applies to that appointment unless the client later requests and approves additional services or time.

    The Social Age reserves the right to change future service rates. Any pricing changes will not alter the price of an already confirmed and paid appointment unless the appointment itself is modified.

    Failed or Incomplete Payments
    If a required payment cannot be successfully processed, the appointment may remain unconfirmed or be canceled. The client is responsible for ensuring that payment information is accurate and current.

    Refunds & Cancellations
    Refund eligibility is determined according to The Social Age's Cancellation & Rescheduling Policy. Payment for a service does not override applicable cancellation, late-cancellation, or no-show terms.

    Receipts
    Clients will receive applicable booking and/or payment confirmation electronically through the booking/payment system.

  • CANCELLATION POLICY

  • The Social Age understands that plans change, especially when health, family, and caregiving needs are involved. At the same time, each scheduled visit reserves dedicated time that may no longer be available to another client.

    Cancellations & Rescheduling
    Appointments may be canceled or rescheduled at least 24 hours before the scheduled start time without a cancellation fee. Any prepaid amount will be refunded or, at the client's request, applied toward a rescheduled visit.

    Late Cancellations
    Appointments canceled less than 24 hours before the scheduled start time are considered late cancellations.

    A late cancellation will result in a charge equal to 50% of the scheduled service cost. If the appointment was prepaid, the remaining 50% will be refunded.

    No-Shows / Unable to Access Client
    If a team member arrives for a scheduled visit and cannot access the client or service location, and reasonable attempts to contact the client or designated contact are unsuccessful, the visit may be considered a no-show.

    No-shows are non-refundable and may be charged 100% of the scheduled service cost.

    If inability to access the client creates a reasonable concern for the client's immediate safety or well-being, The Social Age may contact the client's emergency contact or emergency services as appropriate.

    Emergencies & Illness
    We understand that emergencies happen. Cancellation fees may be waived for circumstances such as hospitalization, sudden illness, family emergencies, severe weather, or other unexpected situations at the discretion of The Social Age.

    Clients are encouraged to contact us as soon as reasonably possible.

    Late Arrivals by the Client
    If a client is unavailable at the scheduled start time, the visit will generally still end at its originally scheduled end time. The full scheduled visit price may still apply.

    Cancellation by The Social Age
    If The Social Age must cancel a visit and cannot provide an acceptable replacement or rescheduled time, the client will receive a full refund or service credit, according to the client's preference.

    Clients will never be charged a cancellation fee when The Social Age cancels the visit.

    Severe Weather & Unsafe Conditions
    The Social Age may cancel or reschedule services when weather, road conditions, unsafe home conditions, or another circumstance makes providing the service unsafe.

    When The Social Age initiates the cancellation for safety reasons, the client will not be charged a cancellation fee.

    Repeated Cancellations
    Frequent late cancellations or no-shows may result in changes to scheduling arrangements, a requirement for additional booking conditions, or discontinuation of recurring services.

  • PERSONAL PROPERTY & VALUABLES

    Clients are encouraged to secure cash, jewelry, financial documents, medications, firearms, and other valuable or sensitive property.The client agrees not to ask team members to perform financial transactions, sign legal documents on the client's behalf, access financial accounts, borrow money, accept significant gifts, or engage in activities outside the agreed service scope.Any concern regarding missing property or inappropriate conduct should be reported promptly to The Social Age.
  • TRANSPORTATION

    Transportation is not included in my current services.
  • Pets

    The client agrees to disclose animals present in the home.The Social Age may request that an animal be secured if a team member reasonably believes the animal presents a safety concern.Pet assistance is limited to tasks agreed upon in advance and does not constitute veterinary care.
  • PRIVACY & CONFIDENTIALITY

    The Social Age will treat personal information obtained in connection with services as confidential and will use or disclose it only as reasonably necessary to provide and administer services, as authorized by the client or authorized representative, as required by law, or when reasonably necessary in an emergency involving health or safety.Authorization to routinely communicate with family members or other third parties is addressed separately in the Family / Authorized Contact form.Photo and media permission is addressed separately and is completely optional.
  • CLIENT RIGHTS

  • Clients receiving services from The Social Age have the right to:

    • Be treated with dignity, respect, and consideration
    • Participate in decisions regarding their services
    • Have personal preferences respected whenever reasonably possible
    • Ask questions about services and fees
    • Express concerns without retaliation
    • Decline a particular activity or service
    • Request changes to services
    • Have personal information handled respectfully and confidentially
    • End services in accordance with this agreement

  • SERVICE TERMINATION

  • The client or The Social Age may discontinue services according to the terms of this agreement.

    The Social Age may discontinue or suspend services when, for example:

    • The client's needs exceed the services The Social Age can safely provide
    • The environment becomes unsafe
    • Requested tasks fall outside permitted service scope
    • There is threatening, abusive, discriminatory, harassing, or unsafe conduct toward staff
    • Required payments remain unpaid
    • Necessary information is intentionally withheld
    • Continued services would otherwise be unsafe, unlawful, or inappropriate

    When reasonably possible, The Social Age will communicate concerns before discontinuing non-emergency services.
