    Parent Participation & Safety Agreement
  • Welcome to Toddle and Talk, a parent/guardian-accompanied play group facilitated by Natalie Cooley, M.A., CCC-SLP at Playful Pathways Therapy.

By signing below, I acknowledge and agree:

    Parent/Guardian Supervision

A parent or legal guardian must remain present and actively supervise their child for the entire session. This is not a drop-off program, childcare service, babysitting service, or one-to-one supervision.

    Nature of the Group

This is a play-based group experience for toddlers and their caregivers. It is not medical care, a diagnostic evaluation, or individualized speech-language therapy. Parents are responsible for deciding whether participation is appropriate for their child.

    Assumption of Ordinary Play Risks

I understand that toddler play and group activities involve normal risks, including slips, falls, bumps, scrapes, collisions with people, furniture, toys, or materials; interactions with other children; choking or mouthing risks; allergic or skin reactions; and exposure to common illnesses.

I voluntarily choose to have my child participate and understand that Natalie Cooley, M.A., CCC-SLP or Playful Pathways Therapy cannot eliminate every risk associated with a shared toddler play environment.

    Health and Safety

I will not bring my child if they are sick or have symptoms that could reasonably expose others to illness and will follow reasonable safety directions during the group. I understand that I am responsible for any personal care or medical needs that my child may require during the class.

    Emergency Care

If an emergency occurs, I authorize Natalie Cooley, M.A., CCC-SLP, Playful Pathways, or any contractors or employees of the aforementioned to contact emergency services and attempt to contact me using the information below. I understand that I am responsible for any medical or emergency-related costs for me or my child.

    Acknowledgment and Release

To the fullest extent permitted by Michigan law, I release Natalie Cooley, M.A., CCC-SLP, Playful Pathways, its facilitators, contractors, and representatives from claims arising from the inherent and ordinary risks of participating in this parent-accompanied play group. Nothing in this agreement is intended to waive any rights that cannot legally be waived.
