This form is for individuals who are already registered to attend the SHPE 2026 National Convention and would like to bring minor family members to Familia Fest.

Before You Begin

Please review the following registration requirements:
- Children under age 3 do not need to be registered and should not be included on this form.
- Minors ages 3–17 must be registered to attend Familia Fest.
  Anyone age 18 or older must have their own National Convention or Familia Fest registration.
- A registered adult may register up to five (5) minors ages 3–17 to attend Familia Fest with them.
- Minors registered through this form must attend Familia Fest with the National Convention attendee submitting this form.
- Children under age 3 do not count toward the five-minor limit.
If another adult family member will be attending Familia Fest with the minors: That adult may complete their own Familia Fest registration and add up to five minors ages 3–17 to their registration. If they do so, you do not need to complete this form.

By submitting this form, you are requesting to register the minor(s) listed on this form to attend SHPE Familia Fest under your SHPE National Convention registration.

Children under age 3 as of 10/31/2026 do not need to be registered and should not be included on this form.
CHAPERONE REQUIREMENT

All persons who will be under the age of 18 as of 10/31/2026 must be accompanied and supervised by a registered parent or legal guardian ("Chaperone") at all times during SHPE Familia Fest.

I understand that I must be fully registered for the SHPE 2026 National Convention in order to register minor(s) through this form. I must attend SHPE Famlia Fest with the minor(s) registered under me and supervise them throughout the event.

A Chaperone may register and supervise up to five (5) minors ages 3-17. Children under age 3 do not require registration and do not count toward the five-minor limit.
ACCESS LIMITATION - FAMILIA FEST ONLY / NO CONVENTION ACCESS

I understand and acknowledge that the Familia Fest registration provided to the minor(s) listed on this form is ONLY for SHPE Familia Fest on Saturday, 10/31/2026 and does NOT provide the minor(s) access to any other portion of the SHPE National Convention, including but not limited to convention sessions, the exhibit hall, career fair, hospitality suites, or any other convention programming.
BEHAVIORAL/REMOVAL CLAUSE

SHPE reserves the right to remove any participant or attendee whose behavior is deemed unsafe, disruptive, inappropriate, or inconsistent with event policies.

I understand that my obligation to supervise the minor(s) registered through this form includes managing their behavior consistent with the Code of Conduct.
MEDIA AND PHOTOGRAPHY RELEASE

I understand that photographs, video recordings, and other media may be captured during this event for promotional, archival, and marketing purposes. I grant SHPE permission to photograph, record, and use the likeness, image, voice, and appearnace of the minor(s) registered through this form during the event in promotional materials without further notice or compensation. I understand that all photographs and recordings become the property of SHPE and may be edited, reproduced, published or distributed for lawful purposes.

If I do not wish for the minor(s) registered through this form to be photographed or recorded, I agree to notify SHPE in writing prior to the event and to make reasonable efforts to avoid designated photography or recording areas.
DATA COLLECTION AND CONSENT

I understand that SHPE will collect and use my personal information (such as name, contact details, and registration information) and that of the minor(s) registered through this form for event registration, communication, safety, and administrative purposes.

I consent to SHPE sharing such information only with authorized event staff, contractors, volunteers, emergency personnel if needed, and service providers as necessary to operate the event. I understand my information and that of the minor(s) registered through this form will not be sold. I understand that reasonable safeguards will be used to protect such information, but no system is completely secure.

I consent to the use of my information and that of the minor(s) registered through this form as described above and understand that providing this information is required to participate in the event.
ASSUMPTION OF RISK AND COVENANT NOT TO SUE

SHPE Familia Fest may include educational demonstrations, hands-on STEM projects and activities, health education, arts and crafts, interactive displays, and other similar activities. These activities and participation in SHPE Familia Fest involve inherent risks including risks of bodily injury, illness, death, or property damage, whether known or unknown.

I understand and acknowledge that participation in SHPE Familia Fest involves inherent risks including, but not limited to the risks described above. I knowingly and voluntarily assume all risks associated with the participation of the minor(s) registered through this form. I have carefully read and fully understand the registration questions and freely accept these risks and waive any rights regarding liability as described above.

I hereby irrevocably covenant and agree not to commence, institute, or prosecute any claim, demand, action, or proceeding of any kind against SHPE arising out of or relating to any matters that I have released, waived, or discharged under this registration agreement and questionnaire.
