I hereby grant Veterans Resource Alliance New England AND Walking With Honor permission to use my likeness in a photograph, video, or other digital media in any and all of its publications, including web-based publications.
I understand and agree that all photos, videos, and any other collected media will become the property of Veterans Resource Alliance New England AND Walking With Honor and may be used in subsequent promotional material(s).
I hereby irrevocably authorize Veterans Resource Alliance New England AND Walking With Honor to edit, alter, copy, exhibit, publish, or distribute these photos for any lawful purpose. In addition, I waive any right to inspect or approve the finished product wherein my likeness appears. I hereby hold harmless, release, and forever discharge Veterans Resource Alliance New England AND Walking With Honor from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.
I HAVE READ AND UNDERSTAND THE ABOVE PHOTO RELEASE. I AFFIRM THAT I AM AT LEAST 18 YEARS OF AGE, OR, IF I AM UNDER 18 YEARS OF AGE, I HAVE OBTAINED THE REQUIRED CONSENT OF MY PARENTS/GUARDIANS AS EVIDENCED BY THEIR SIGNATURES BELOW.
In consideration of being allowed to participate in the Veterans Resource Alliance New England AND Walking With Honor’s annual “United We Stand” Kickball event, sponsored by Delta Dental Arena, in accordance with the Town of Hooksett, New Hampshire and Merrimack County, I (the undersigned) acknowledge that I am fully aware of the risks and hazards connected with the Activity, including physical injury, and I VOLUNTARILY ASSUME FULL RESPONSIBILITY FOR ANY RISKS OR LOSS, PROPERTY DAMAGE, OR PERSONAL INJURY, that may be sustained by me (the undersigned), or loss or damage to property owned by me, as a result of participation in the Activity. I hereby RELEASE, WAIVE, DISCHARGE, AND COVENANT NOT TO SUE, the Town, its elected and appointed officials and officers, servants, agents, and employees, and the DDA, its officers, servants, agents, and employees (hereinafter referred to as RELEASEES) from any and all liability, claims, demands, actions and causes of action whatsoever arising out of or related to any loss, damage, or injury, that may be sustained by me, or to any property belonging to me, while participating in the Activity. I hereby further RELEASE, WAIVE, DISCHARGE, AND COVENANT NOT TO SUE, Delta Dental Arena, OR Veterans Resource Alliance New England, its members, volunteers, employees, and any other recognized officials corroborated by either party, OR Walking With Honor, its members, volunteers, employees, and any other recognized officials corroborated by either party, OR the NH Fisher Cats, its members, volunteers, employees, and any other recognized officials corroborated by either party. I hereby further agree that this Agreement shall be constructed in accordance with the laws of the State of New Hampshire. In signing this release, I acknowledge and represent, that I have read the foregoing Liability Waiver and Hold Harmless Agreement, that I understand it, and that I sign it voluntarily as my own free act and deed; that no oral representations, statements or inducements, apart from the foregoing written agreements have been made; and that I EXECUTE THIS RELEASE FOR FULL, ADEQUATE, AND COMPLETE CONSIDERATION FULLY INTENDING TO BE BOUND BY SAME.
