UAC Christmas Hampers of Hope Application Form 2026

ELIGIBILITY REQUIREMENTS:

1. Residency:
- You must be a resident of Markham, Stouffville or Richmond Hill.
- Exception: Regular Master's Pantry clients in York Region and Unionville Alliance members are eligible even if they now reside outside of York Region.
2. Hamper Exclusivity:
- You may not receive a Christmas hamper from another church, agency, or organization.
- We will cross-reference with other churches and agencies to ensure each household receives only one hamper. Please note that we only allow one hamper per household.
3. Application Denial:
- Your application will be denied if you do not meet the eligibility requirements.
EACH HAMPER PER HOUSEHOLD CONTAINS:
- Enough food for 2-3 days.
- A Christmas meal (choice of frozen turkey, chicken, or no meat). Please note that our meat options are not halal.
- Gift cards to help with toys and gifts for your family.
IMPORTANT NOTES BEFORE BEGINNING YOUR APPLICATION:
- The name provided in the application is the name you must use when picking up your hamper. Identification will be required to collect your hamper. Please be mindful to include your legal name that is on your ID on the application. If someone else will be picking up your hamper, please provide their name at the bottom of this form.
- You must arrive to pick up your hamper at your scheduled time. If you arrive before your designated pick-up time, you will be asked to return at your scheduled time. Hampers will not be given out ahead of your designated pick-up time. Please keep this in mind when selecting your pick-up time.
- The deadline for changes to pick-up times and dates is Thursday, December 10th. No changes can be made after this date.
- Please ensure that the phone number listed in your registration is correct in the event we may need to contact you about your hamper.

The following information will be used to help ensure that we can provide the appropriate amount of food and gifts for your entire household. Note: Only one hamper per household.

Pick Up Instructions

Your hamper will be available for curbside pick up. Please choose a 15-minute time slot for either Friday, December 18th or Saturday, December 19th.

You can reschedule your pick-up no later than Thursday, December 10.

Existing Master's Pantry Clients should pick up on Saturday, December 19th. Hampers will not be available for pick up on the 18th for regular pantry clients.
