Welcome to Lifeline All Stars! Our program gives girls in grades K–12 an opportunity to learn cheer fundamentals, dance, teamwork, confidence, sportsmanship, and performance skills in a positive youth-sports environment.

PARTICIPATION ACKNOWLEDGMENT & RELEASE
I understand that cheerleading, dance, conditioning, tumbling, stunts, sports events, and related activities involve physical activity and risks of injury. I voluntarily allow my child to participate. I agree to follow program safety rules and to inform coaches of relevant restrictions. To the extent permitted by law, I release and hold harmless Lifeline Youth Sports, Lifeline All Stars, their staff, coaches, volunteers, and facility partners from claims arising from ordinary risks of participation, except where liability cannot legally be waived.

This packet is intended as a practical program registration form and is not a substitute for legal advice. The organization may wish to have its final waiver language reviewed by a Georgia attorney.

ATHLETE & FAMILY COMMITMENT
- Athletes are expected to arrive on time, prepared, and dressed appropriately for practices, games, performances, and team events.
- Athletes should treat teammates, coaches, officials, opponents, staff, and families with respect.
- Parents/guardians should communicate attendance issues as early as possible. Consistent attendance is important because cheer routines depend on the full team.
- Unsafe behavior, bullying, fighting, harassment, or repeated disrespectful conduct may result in removal from an activity or additional program action.
- Families are responsible for caring for issued uniform items and following coach instructions for game-day/performance appearance.
- Program schedules, performance opportunities, competition options, and team assignments may vary by age/grade, participation, readiness, and event availability.

WEEKLY PAYMENT POLICY
Weekly Fee: $25 per week
Payment Due: Every Sunday by 9:00 PM
Late Payments: A payment link will be sent to parents/guardians when weekly fees are overdue.
Late Fee: An additional $5 late payment fee will be added when weekly fees are paid after the Sunday 9:00 PM deadline.
