By signing this waiver, I, the undersigned parent or legal guardian, acknowledge and agree on behalf of myself and the minor named in the form as follows:

1. Assumption of Risk: I understand that participation in the event(s) involves risks, including physical injury, illness, or property damage. I voluntarily assume all such risks.

2. Release of Liability: I release BAPS and its affiliates from any liability, claims, or actions arising out of any loss, damage, or injury sustained during the event(s), whether caused by negligence or otherwise.

3. Consent to Emergency Medical Treatment: I authorize BAPS to obtain emergency medical treatment for the minor and agree to be responsible for any costs incurred.

4. Photo/Video Release: I consent to the use of photographs, videos, or other media of the minor taken during the event(s) for promotional or informational purposes by BAPS.
