I understand that Georgia Career Institute will cease all educational operations at its campus effective October 1, 2026. As a result, the school cannot complete my remaining clock-hour instruction at this facility.

I acknowledge that I have been officially notified that Georgia Career Institute will permanently cease educational operations on October 1, 2026.

I further acknowledge that Georgia Career Institute has presented me with formal Teach-Out options through agreements with the following institutions: Beaver Beauty Academy, Empire Beauty School (Morrow, Atlanta, and Lawrenceville Campuses), ProWay Hair School, and Elaine Sterling Institute.

These formal Teach-Out options allow me to transfer 100% of my earned clock hours and complete my remaining program requirements without tuition charges above the tuition amount established in my GCI Enrollment Agreement.

I acknowledge that I have been offered the formal Teach-Out options described above and voluntarily decline to participate in those options.

By declining this teach-out option, I certify that I understand the following statements:

I understand that my enrollment at Georgia Career Institute will end as a result of the institution's closure. By declining GCI's formal Teach-Out options, I will be responsible for identifying and enrolling with another institution if I wish to continue my education outside of the formal Teach-Out arrangements.
I understand that I may review information regarding potential eligibility for a federal Closed School Loan Discharge through the U.S. Department of Education. Eligibility is determined based on my individual circumstances and applicable federal requirements, and is not determined by Georgia Career Institute. https://studentaid.gov/manage-loans/forgiveness-cancellation/closed-school
I understand that my final earned clock hours and academic records will be archived in accordance with applicable record-retention requirements and that I will receive an official transcript upon my withdrawal.

By signing below, I acknowledge that I have reviewed the formal Teach-Out options provided by Georgia Career Institute and voluntarily decline to participate in those options. I understand that I may independently seek enrollment at another institution and that acceptance of my completed clock hours, tuition arrangements, financial aid eligibility, program placement, and other enrollment requirements will be determined by the receiving institution. I also understand that my decision to decline GCI's formal Teach-Out options does not alter any rights or obligations that may otherwise exist under applicable federal or state law.
