This form is required for adult participants and staff to attend the University Catholic fall Awakening Retreat from October 30-November 1, 2026, at Deer Run Camps & Retreats (3845 Perkins Rd, Thompson's Station, TN 37179).

REGISTRATION CLOSES AT NOON ON TUESDAY, OCTOBER 27, 2026. If the deadline has passed, please contact the UCat Diocesan Office via phone or email for approval before completing this form.

I agree to defend, protect, indemnify and hold harmless the Office of Faith Formation, Diocese of Nashville against and from all claims arising from my negligence or fault which arise out of my participation in this activity including but not limited to any personal injury, property damage, disability, death, and sickness or disease including, without limitation, any communicable disease. Additionally, I agree to protect, defend, hold harmless and fully indemnify the Office of Faith Formation, Diocese of Nashville for any claim or cause of action whatsoever arising out of this activity which takes place during the above identified dates that is brought against the Office of Faith Formation, Diocese of Nashville by me or my family members whether such claim arises from the alleged negligence of the Office of Faith Formation, Diocese of Nashville, its employees or agents or my negligence. If any portion of this agreement is held invalid, it is agreed that the balance thereof, shall continue in full legal force and effect.
I hereby authorize The Diocese of Nashville, The Tennessee Register, and its agents to utilize my photographic image for the specific purpose of publication of all promotional materials, websites, and official social media accounts. I understand that I will receive no compensation should any photograph of me be used.
I agree to abide by any/all policies and rules established for this event. Should I not be able to maintain the guidelines and expectations for this event, I understand there will be consequences for my actions, which could include my being asked to leave the event.

Transportation

University Catholic provides limited transportation via bus from Frassati House (2004 Terrace Pl, Nashville, TN 37203) to Deer Run and back. Spots are available on a first-come, first-served basis. Confirmed spot and details about logistics will be shared directly with bus riders. University Catholic will NOT organize any other transportation.

The bus will LEAVE Frassati House at 3 PM on Friday, October 30.
The bus will RETURN to Frassati House around 3 PM on Sunday, November 1.

If you are organizing your own transportation, please arrive to Deer Run by 4 PM on Friday, October 30. The retreat will conclude around 2 PM on Sunday, November 1.

The cost to attend an Awakening Retreat as a first-time retreat participant is $100 and includes all meals and lodging for the duration of the retreat.

The cost to return to an Awakening Retreat as student staff is $60 and includes all meals and lodging for the duration of the retreat.

The payment link will be

- emailed to the email address you provided on this registration form,
- and available at www.universitycatholic.org/awakening.
Payment: If I submit this registration form, but do not submit payment, I am not fully registered, and my spot will not be guaranteed. Payment for my registration fee must be received by the Diocese of Nashville's Office of Faith Formation for University Catholic by noon on Tuesday, October 27, 2026.
Reschedule: If I wish to switch my registration to a the spring Awakening Retreat weekend (Date TBD), I must request to be rescheduled by contacting the Diocese of Nashville's Office of Faith Formation for University Catholic by noon on Tuesday, October 27, 2026. After this, the registration fee will not be transferable.
Refund: If I am no longer able to attend the Awakening Retreat, I must request a refund by contacting the Diocese of Nashville's Office of Faith Formation for University Catholic by noon on Tuesday, October 27, 2026. After this, the registration fee will not be refundable.
