  • Patch Test

    Patch testing is recommended before tint, hybrid stain, henna, and lamination services. A patch test is a quick safety check where a small amount of the product(s) planned for your service is applied behind the ear or on the inner arm, depending on the product being tested, at least 48 hours before your appointment. You’ll monitor the area over the next 48 hours for signs of a reaction such as redness, itching, swelling, or burning.

    • I understand that SugarbyLexa recommends a patch test 48 hours before tint, hybrid stain, henna, or lamination services.

    • I understand that a patch test may help identify a possible sensitivity or allergic reaction but cannot guarantee that a reaction will not occur.

    • If I choose not to complete a recommended patch test, I understand that I am choosing to proceed at my own risk and accept responsibility for any reaction that may occur.

    • I understand that SugarbyLexa is not responsible for reactions that may occur when I choose to proceed without a recommended patch test.

  • Policy Acknowledgement

    By booking, clients acknowledge and agree to the following:

    • Late cancellations, reschedules within 48 hours, and no-shows may be charged up to 100% of the service booked.
    • Arriving more than 15 minutes late may be treated as a no-show and charged accordingly.
    • All services are final sale. No refunds will be issued.
    • Clients are responsible for providing accurate information on their Brow Client Intake Form and must report any changes to medications, skincare, allergies, health, or recent treatments before their appointment.
    • SugarbyLexa reserves the right to refuse service, modify a service, or discontinue treatment at any time for safety, policy, or conduct reasons.
    • Clients are responsible for following all pre-care and aftercare instructions provided for their brow service.

  • Client Acknowledgment

    • I understand that the brow service I receive will depend on what I have booked and may be adjusted if needed based on my skin, brow hair, or safety.
    • I understand that although every precaution is taken, irritation, sensitivity, allergic reaction, or other unexpected reactions may still occur.
    • I confirm that I have provided accurate information about my health, medications, skincare, allergies, recent treatments, and previous reactions, and I will let SugarbyLexa know if anything changes before my appointment.
    • I understand that my service may be modified or stopped if continuing is not considered safe or suitable.
    • I accept the risks associated with the service being performed and release and hold harmless SugarbyLexa from liability arising from the treatment.
    • I confirm that the information provided in this form is accurate and truthful to the best of my knowledge.
    • I confirm that I have reviewed and agreed to SugarbyLexa’s Policies and Terms of Service.
