DI Playroom – Parent/Guardian Waiver & Acknowledgement
By registering for and participating in DI Playroom at Dance Invasion, I acknowledge and agree to the following:
Parent Supervision: DI Playroom is a parent/caregiver-supervised open play program and is not childcare. I understand that I am responsible for actively supervising my child(ren) at all times. A Dance Invasion staff member will be present; however, staff are not responsible for supervising my child(ren).
Assumption of Risk: I understand that participation in open play and the use of toys, equipment and studio spaces involve inherent risks, including but not limited to slips, trips, falls, collisions and other injuries that may occur during normal play. I voluntarily accept these risks on behalf of myself and my child(ren).
Release of Liability: To the extent permitted by law, I release and hold harmless Dance Invasion, its owners, employees, instructors and representatives from claims arising from the ordinary risks associated with participation in DI Playroom, except where liability cannot legally be excluded or waived.
Health & Safety: I agree not to attend or bring a child who is unwell or experiencing symptoms of a contagious illness. I agree that my child(ren) and I will follow all posted Playroom and studio rules and use toys and equipment appropriately. Personal Belongings: I understand that Dance Invasion is not responsible for lost, stolen or damaged personal belongings.
Emergency Care: In the event of an emergency, I authorize Dance Invasion staff to contact emergency services and provide reasonable assistance until emergency personnel arrive. I understand that I am responsible for any resulting medical costs.
By signing below, I confirm that I am the parent/legal guardian of the participating child(ren), or otherwise have authority to sign on their behalf. I have read and understood this waiver and voluntarily agree to its terms.
