BY ACKNOWLEDGING AND SIGNING BELOW, I AM DELIVERING AN ELECTRONIC SIGNATURE THAT WILL HAVE THE SAME EFFECT AS AN ORIGINAL MANUAL PAPER SIGNATURE. THE ELECTRONIC SIGNATURE WILL BE EQUALLY AS BINDING AS AN ORIGINAL MANUAL PAPER SIGNATURE.

By signing this waiver form, I grant permission for the child named above to participate in and engage in the James S. Kunzman Foundation Free Youth Baseball and Softball Camp. My child is physically and mentally able to participate in these activities, unless I have already discussed it with one of the coaches. I acknowledge that there are certain risks involved in said activities, and have discussed them with my child if necessary. 

I release the James S Kunzman Foundation (including it'sofficers, board, advisory board, affiliates, etc.), Goddard High School/NMMI/Roswell High School/Camp instructors, it's affiliates, volunteers, and employees of all responsibilities for any injuries, to body or property, which may occur to my child during the course of these activities. In the event of any emergency in which I, or the alternate contact, cannot be reached, I authorize the adult leaders to make medical decisions for my child and to administer first aid if necessary. 

I further agree to indemnify and hold harmless the James S Kunzman Foundation (including it'sofficers, board, advisory board, affiliates, etc.), Goddard High School/NMMI/Roswell High School and it's affiliates, volunteers, and employees of any and all claims arising from the participation of my child in activities or as a result of injury or illness of my child or mine during such activities.

Further, I, the undersigned, grant permission for the use of my or my child’s photo, image, and likeness in any photos, videos, or other media taken during this event. I understand these images may be used for promotional, educational, or marketing purposes in print or digital media. I waive any right to inspect or approve the final use of these materials and release the organizers from any claims related to the use of these images. 

I represent that I am the participant, or the parent/guardian of {participantsName}, who is under 18 years of age. I have read the permission/waiver form and I am fully aware of it's contents. I give permission for the child named above to participate fully in the activities of James S. Kunzman Foundation Free Youth Baseball and Softball Camp.
