I, ________________________________, understand and agree that by providing my own parts for service at Precision AutoTek, the following terms and conditions apply:

1. I affirm that the parts I am supplying are new, genuine, and suitable for their intended purpose. I understand that Precision AutoTek cannot guarantee the quality, safety, or compatibility of parts they do not directly source.

2. I agree to pay a non-refundable shop supply fee of $75 to cover the time and resources needed for the technician to receive, inspect, test, and install my supplied parts. I understand this fee applies regardless of the outcome of the service.

3. I understand that any verbal or written estimates provided by Precision AutoTek prior to having physical possession of my supplied parts were for reference only. Final labor costs are subject to change after the technician has had the opportunity to fully inspect the parts and assess fitment and compatibility.

4. I agree that the final price for service will be determined after Precision AutoTek has received and inventoried all necessary parts and may differ from previous quotes if additional work is required due to the condition, completeness, or suitability of the parts I supplied.

5. Precision AutoTek's warranty coverage on customer-supplied parts is limited to 30 days from the date of service. This warranty covers defects in workmanship only, not the parts themselves.

By signing below, I agree to these terms and release Precision AutoTek from all liability related to the performance or suitability of my supplied parts. I affirm that I have been given the opportunity to have Precision AutoTek supply the parts for my repair at their standard retail pricing.
