Sponsorship Options

There are three different sponsorship options to choose from. Please read each option below & select the one that works best for you. Once your form is submitted, selections cannot be changed.
OPTION 1: Sponsor will be responsible for shopping, wrapping, and delivering all wish list items purchased to the CHD Office by Tuesday, December 1st.

$200.00 per child, per wish list

OPTION 2: Sponsor is responsible for shopping, shipping and/or delivering all wish list items purchased to the CHD Office by Friday, November 20th. CHD Staff will take care of the wrapping!

OPTION 3: Sponsor will make a direct donation on our website of $250.00 per child, per wish list. CHD Staff will do the shopping and wrapping for you!

For Option 3, please submit your donation on our website or mail a check no later than Friday, October 30th.
