Registration Form for Van Horn Pet Clinic on Tuesday, October 6, 2026
Every pet needs to be individually registered, do not put multiple pets on one form. This clinic is ONLY being held on October 6, 2026. This is a surgical clinic - do not register if you only want vaccinations. Services are being performed at Fort Davis Veterinary Services in Fort Davis. Registered/confirmed pets will meet in front of City Hall (parking lot) at 6:45 am on October 6. Pets should have no food that day. To complete the registration process you will sign an authorization form at drop off. OTAT-West Texas will drive the pets from Van Horn to Fort Davis Veterinary Services. OTAT-West Texas will provide crates for dogs based upon the weight information entered in the registration form. If you are registering a cat, you must provide your own carrier. OTAT-West Texas will return the pets to the City Hall parking lot at 6:45 pm on October 6. Pain medication and aftercare instructions will be provided. Pets may not be substituted. By proceeding with registration you commit to having your pet at the City Hall parking lot at 6:45 am and returning at 6:45 pm for pickup. This clinic is free.
