Purpose & Scope. I desire to access and use the Family Life Center and/or The Loft. This could include fitness classes, gymnasium, equipment, locker rooms, showers, and related facilities and programs operated by First Baptist Amarillo.

Acknowledgment of Risks. I understand that fitness activities inherently involve risks including falls, collisions, equipment malfunction, muscle strains, fractures, cardiac events, dehydration, and other serious injury or death.

Assumption of Risk. I voluntarily assume all risks associated with participation.

Waiver & Release. I waive and release the Church from liability for injury, illness, death, or property damage, except as prohibited by law.

Indemnification. I agree to indemnify and hold harmless the Church from any related claims.

Medical Fitness & Consent. I represent that I am physically able to participate and consent to emergency medical treatment.

Personal Property. I understand the Church is not responsible for lost or stolen items.

I grant permission for First Baptist Amarillo to take and use photographs and videos of me/my child for social media, website, printed materials, internal communication, and promotional purposes.

I understand images may be used without compensation and may be edited. I may revoke permission in writing; revocation does not affect materials already produced.
