What do we mean by a chronic or long-term condition?
It is a health condition that lasts for a long time and may require ongoing care, treatment or regular management. Examples include high blood pressure, diabetes, asthma, sickle cell disease, heart disease, kidney disease, epilepsy, arthritis and cancer.
We're building Auxichron Health, a new healthcare initiative focused on improving how people living with long-term health conditions manage their care. As part of our early research, we want to learn from people who live with a chronic condition or support someone who does.
There are no right or wrong answers. We simply want to understand your honest experiences, what works well, what you find difficult and what you would like to see improved.
You can fill on behalf of someone you care for (e.g sibling, child, parent, etc) but please indicate this in the first question only. For all other questions, we need the information of who you care for, not yours.
This survey takes about 5–7 minutes.
Thank you for helping us learn!
