INFORMED CONSENT, ASSUMPTION OF RISK & RELEASE OF LIABILITY
Goalkeeper Thanksgiving Experience 2026
November 23–25, 2026 | Airport Lakes Park, Orlando, Florida
I understand that participation in goalkeeper training, Goalkeeper Wars, soccer competitions, and other soccer-related activities involves physical activity, including but not limited to running, diving, jumping, kicking, contact with soccer balls, physical contact or collisions with other participants, contact with goals or other equipment, and competitive play. I understand that these activities involve inherent risks of injury.
By registering myself or my child for the Goalkeeper Thanksgiving Experience 2026, I acknowledge and agree that:
• I certify that I/my child is physically able to participate in the activities associated with this event.
• I voluntarily assume the inherent risks associated with participation.
• I understand that it is my responsibility to inform the coaching staff of any medical condition, injury, allergy, medication, or physical limitation that may affect safe participation.
• To the fullest extent permitted by applicable law, I release and hold harmless Orlando Goalkeeping LLC, David Palomino, Nelson Ramos, participating coaches, event staff, organizers, sponsors, volunteers, and the facility owners/operators from claims arising from participation in the event, except to the extent such claims cannot legally be waived or released.
This acknowledgment applies to all event-related activities, including goalkeeper training sessions, demonstrations, warm-ups, competitions, Goalkeeper Wars, and any other activities associated with the Goalkeeper Thanksgiving Experience.
EMERGENCY MEDICAL AUTHORIZATION
In the event of an injury, illness, or medical emergency, I authorize Orlando Goalkeeping LLC and authorized event staff to obtain reasonable emergency medical assistance for me/my child if I cannot be reached immediately or if circumstances require immediate attention.
I understand that I am responsible for any medical expenses incurred as a result of such treatment.
MEDIA RELEASE
I grant Orlando Goalkeeping LLC, David Palomino, Nelson Ramos, and their authorized event partners permission to photograph and/or record me/my child during the Goalkeeper Thanksgiving Experience.
I authorize photographs and recordings from the event to be used for legitimate promotional, marketing, educational, website, advertising, and social-media purposes without compensation.
ACKNOWLEDGMENT
By signing below, I acknowledge that I have read and understood this Informed Consent, Assumption of Risk, Release of Liability, Emergency Medical Authorization, and Media Release.
If the participant is under 18 years of age, I certify that I am the participant’s parent or legal guardian and have authority to provide this consent on their behalf.
