Membership Cancellation Request
This form is for canceling your month-to-month membership with 10th Planet Jiu Jitsu San Mateo.
This form is only for monthly paid memberships. Canceling a paid-in-full plan (6-month or 12-month) would amount to a refund request, and paid-in-full memberships are non-refundable — so this form can't be used for those. If you're on a paid-in-full plan and have questions, please contact us directly rather than submitting this form.

Cancellation Policy
- No refunds Forgetting to submit this form, or not attending class, does not cancel your membership and does not entitle you to a refund. You will continue to be billed normally until you submit a cancellation request through this form.
- Cancellation is only available for month-to-month memberships. Paid-in-full memberships are non-refundable and not eligible for early cancellation.
- Your cancellation request date must be at least 7 days in advance, as per your agreement to provide 7 days' notice. This form won't let you select a date closer than that, so plan your request accordingly.
- Any payment scheduled before your cancellation request date is still charged as normal and is non-refundable.
- Cancellation ends your membership — it does not pause it. If you'd like to train with us again in the future, you'll need to sign back up as a new member, which means paying the onboarding fee again in addition to whatever membership rates are current at that time..
How Cancellation Works
Your cancellation date is the day your membership ends and your access stops — this is not just a notice date, it's your last day of access.
- Any payment scheduled before your cancellation date is processed as normal and is non-refundable.
- Any payment scheduled on or after your cancellation date is removed — you won't be charged for it.
- Cancelling early does not entitle you to a refund or a prorated credit for unused time.
If you'd like to get full use of what you've already paid for, set your cancellation date to your next upcoming payment date — that way your access runs all the way through the last day covered by your most recent payment, and you're not charged again after that.
Example: Your billing date is the 5th of each month, and you paid on September 5th. If you set your cancellation date to October 5th, that payment is removed and you train normally all the way through October 5th — full use of what you already paid for, no extra charge. If instead you set your cancellation date to September 15th, your access ends that day — and since you already paid through your next billing cycle, you won't be refunded or credited for the remaining time. Setting an earlier date is your choice to make, but it means forfeiting those unused days.
