HBCU SOUL is committed to creating programs, resources, and experiences that strengthen belonging, leadership, health and wellness, advocacy, community connection, and student success among LGBTQIA+ students, alumni, and communities connected to Historically Black Colleges and Universities (HBCUs).
Before beginning this survey, please review the information below so you can make an informed decision about participating.
Purpose of This Evaluation
The purpose of this survey is to help HBCU SOUL understand participants' experiences and evaluate the reach, quality, and impact of our programs.
Information collected through this survey may be used to:
- Assess participant experiences and program satisfaction
- Measure changes in knowledge, confidence, belonging, leadership, advocacy, health-resource awareness, and community connection
- Identify community needs, barriers, and opportunities
- Improve current and future HBCU SOUL programs
- Develop new programs, services, and partnerships
- Evaluate HBCU SOUL's overall impact
- Prepare reports for organizational leadership, universities, community partners, sponsors, and funders
- Communicate aggregated program outcomes through impact reports, presentations, grant applications, and other evaluation materials.
What Participation Involves
You will be asked questions about your experiences, perspectives, knowledge, confidence, connections, and participation in HBCU SOUL programming.
Some questions may address topics such as LGBTQIA+ identity, campus experiences, mental health and social support, sexual health knowledge, HIV/STI prevention, healthcare access, leadership, advocacy, and community belonging.
You may also be invited to complete follow-up evaluations so that HBCU SOUL can better understand how participant experiences and outcomes change over time.
Voluntary Participation
Your participation is completely voluntary.
You may choose not to participate, decline to answer any optional question, or stop completing the survey at any time without penalty.
Your decision about whether to participate will not affect your ability to participate in HBCU SOUL programs or access services or resources that would otherwise be available to you.
Potential Risks or Discomfort
Some questions may involve personal or sensitive subjects and may cause minor emotional discomfort. You are encouraged to skip any optional question you do not wish to answer.
This survey is not intended to provide medical, mental health, legal, or other professional advice or services.
Potential Benefits
You may not receive a direct personal benefit from completing this survey. However, your feedback may help HBCU SOUL improve programs, identify community needs, strengthen resources and partnerships, and better serve current and future participants.
Privacy and Use of Information
HBCU SOUL will make reasonable efforts to protect the privacy of information collected through this evaluation.
Evaluation results may be analyzed and reported in aggregate or de-identified form. This means reports may describe overall patterns, percentages, themes, or participant experiences without intentionally identifying individual respondents.
Open-ended responses may also be analyzed to identify common themes and experiences. HBCU SOUL will make reasonable efforts to remove personally identifying information before using individual comments or stories in external evaluation materials.
Contact information collected for follow-up communication, interviews, or focus groups will be used for those purposes and, where practicable, maintained separately from evaluation responses.
Please avoid including names or other unnecessary personally identifying information in open-ended responses.
Follow-Up Participation
You may be asked whether you are willing to participate in future follow-up surveys, interviews, or focus groups.
Agreeing to complete this survey does not require you to participate in any future activity. Additional participation remains voluntary.
