Jazz Matters, Inc. is inviting wellness professionals, mental-health professionals, musicians and creative practitioners, veteran-serving organizations, community-resource providers, vendors, sponsors, and other partners to participate. This form is designed to learn how you would like to participate, what resources you would need, and what compensation or partnership arrangement would make participation feasible.

The event will take place on Sunday, November 8, 2026, from 12 noon to 2 p.m. at the Elysium Event Center in College Park, Georgia.
