By signing this form, I acknowledge that I desire to work as a volunteer for Be the Church Outreach and engage in the activities related to the outreach effort.
Release and Waiver: I do hereby release and forever discharge and hold harmless Be the Church Outreach and Sixth Mount Zion Baptist Church and it successors and assigns from any and all liability, claims, and demands of whatever kind or nature, either in law or in equity, which arise or may hereafter arise from volunteering with Be the Church Outreach. I understand that this Release discharges Be the Church Outreach and Sixth Mount Zion Baptist Church from any liability or claim that I may have against Be the Church Outreach and participants. With respect to any bodily injury, personal injury, illness, death, or property damage that may result from volunteering with Be the Church Outreach, whether caused by the negligence of Be the Church Outreach and Sixth Mount Zion Baptist Church or its officers, directors, employees, or agents or otherwise will be held harmless. I also understand that Be the Church Outreach and Sixth Mount Zion Baptist Church do not assume any responsibility for or obligation to provide financial assistance or other assistance.
Medical Treatment: I do hereby release and forever discharge Be the Church Outreach and Sixth Mount Zion Baptist Church from any claim whatsoever which arises or may hereafter arise on account of any first aid, treatment, or service rendered in connection with volunteering with Be the Church Outreach.
Assumption of the Risk: I understand that volunteering may include construction, loading and unloading, and transportation to and from the work sites. I hereby expressly and specifically assume the risk of injury or harm and release Be the Church Outreach and Sixth Mount Zion Baptist Church from all liability for injury, illness, death, or property damage resulting from volunteering.
Insurance: I understand that Be the Church Outreach does not carry or maintain health, medical, or disability insurance for any volunteer.
Photographic Release: I do hereby grant and convey unto Be the Church Outreach and Sixth Mount Zion Baptist Church all rights, title, and interest in any and all photographic images and video or audio recordings made by Be the Church Outreach and Sixth Mount Zion Baptist Church while volunteering with Be the Church Outreach, including, but not limited to, any royalties, proceeds, or other benefits derived from such photographs or recordings.
