Use this form to request an inspection to clear an existing violation or to schedule a fire escrow inspection from the Department of Licenses and Inspections(L&I).
This form is intended for inspections conducted by the Inspections, Safety, & Compliance (ISC) Division of L&I. If you are requesting an inspection from the Quality of Life (QOL) Division, please return to the previous page and select the correct option.

Inspections associated with issued construction permits can be requested via eCLIPSE or by using the Interactive Voice Response (IVR) by calling 215-255-0404.

If you are trying to submit a complaint, you can call 311 or (215) 686-8686 if you are outside of Philadelphia or you can use 311's webportal.
