Event Safety, Allergy & Photography Acknowledgment
By submitting this registration and attending this event, I acknowledge and understand the following:
Toys & Small Items: Participants may receive or have access to sensory toys, small toys, novelty items, and other take-home items. Some items may contain small parts that could present a choking hazard and may not be appropriate for all ages or abilities. I am responsible for determining whether an item is safe and appropriate for my child and for providing appropriate supervision during the event and after taking items home.
Food & Allergens: Candy, chocolate, food, and other treats may be distributed during the event. These items may contain or have come into contact with common allergens, including but not limited to peanuts, tree nuts, milk, eggs, wheat, soy, and other ingredients. UnPuzzled Parents Connect, Waverly Activity Center, and participating partners cannot guarantee that any food or treat is allergen-free or has been prepared, packaged, or handled in an allergen-free environment. I am responsible for reviewing all packaging, ingredients, and allergy information before allowing my child to consume any food or treat.
Photography & Video: Photography and/or video recording may take place during this event. By attending, I understand that photographs or video may be taken in event spaces and may be used by UnPuzzled Parents Connect and/or Waverly Activity Center for purposes including social media, websites, newsletters, promotional materials, grant reporting, community outreach, and documentation of the event.
Acknowledgment: By submitting this registration, I confirm that I have read and understand the information above. I accept responsibility for determining the suitability of toys, sensory items, food, candy, and treats for my child and understand that photography and video may occur during the event.
