1. Evaluation Format and Purpose
The Evaluation consists of one initial individual assessment (60 minutes) and one group evaluation session (90 minutes). The sessions take place after school or on weekends at the Nexus fields, located at 2045 Reflection Bay Drive, Pearland, Texas 77584.
The evaluation allows Nexus to assess the Player’s current futbol development and determine the appropriate next step. Completing the evaluation does not guarantee an invitation to submit the Online Application for Admission or admission to Nexus Futbol Academy.
2. Evaluation Fee
The evaluation fee is $150 and must be paid when this form is submitted. The evaluation fee is nonrefundable.
3. Scheduling, Rescheduling, and Cancellations
After registration and payment are received, Coach Pol will contact the family to coordinate the evaluation session.
One reschedule is permitted when at least 24 hours’ notice is provided. Cancellations made with less than 24 hours’ notice and missed sessions without notice will result in forfeiture of the session. Scheduling a replacement session will require an additional evaluation fee.
Nexus may reschedule an evaluation because of weather, field conditions, safety concerns, or other circumstances outside its control. When Nexus reschedules a session, the family will not be charged an additional fee.
4. Attendance and Conduct
The Player should arrive on time with appropriate training attire, footwear, water, and any necessary equipment. The Player is expected to follow all instructions and conduct themselves respectfully and safely.
Nexus may end the Player’s participation if their conduct is unsafe, disruptive, or inappropriate. Removal for conduct reasons will not result in a refund.
5. Health Information and Emergency Care
The Parent/Guardian confirms that the Player is physically able to participate and has disclosed any known injuries, medical conditions, allergies, medications, or physical limitations that may affect participation.
If an injury or medical emergency occurs and the Parent/Guardian cannot be reached, the Parent/Guardian authorizes Nexus staff to provide basic first aid and obtain reasonably necessary emergency medical care. The Parent/Guardian is responsible for any resulting medical expenses.
6. Assumption of Risk, Release, and Indemnity
The Parent/Guardian understands that futbol training and athletic participation involve inherent risks, including physical exertion, falls, collisions, contact with other players, equipment-related injuries, and other injuries that may occur during athletic activity.
The Parent/Guardian voluntarily assumes these risks on behalf of the Player. To the fullest extent permitted by law, the Parent/Guardian releases and holds harmless Nexus Futbol Academy, the Nexus Academy Foundation, and their directors, officers, employees, coaches, agents, volunteers, affiliates, and facility providers from claims arising from the Player’s participation, except where caused by gross negligence or willful misconduct.
The Parent/Guardian also agrees to indemnify and hold these parties harmless from claims or damages resulting from the Player’s intentional misconduct or failure to follow safety instructions.
7. Limitation of Liability
Nexus is not responsible for lost, stolen, or damaged personal belongings. To the fullest extent permitted by law, Nexus’s total liability related to the evaluation will not exceed the amount of the evaluation fee paid, except where such a limitation is prohibited by law.
8. Photography and Video
Nexus may photograph or record the evaluation for internal coaching, assessment, and player-development purposes. Photographs or videos will be used for promotional purposes only when the Parent/Guardian provides separate permission below.
9. Accuracy of Information and Payment Authorization
The Parent/Guardian confirms that the information provided in this form is accurate and complete and that they are authorized to use the selected payment method.
10. Governing Law and Severability
This Agreement is governed by the laws of the State of Texas. If any provision is found to be unenforceable, the remaining provisions will continue in effect.
