PARTICIPATION WAIVER & MEDIA RELEASE
I understand that participation in dance and movement activities involves physical activity and carries the possibility of injury. I voluntarily participate, or give permission for my minor child to participate, in the TAAL Garba Workshop and assume the ordinary risks associated with participation.
To the fullest extent permitted by law, I release and hold harmless TAAL School of Dance & the Indian Arts, its owners, instructors, employees, independent contractors, and representatives from claims arising from the ordinary risks of participation, except where liability cannot legally be waived.
I certify that I, or my minor child, am physically able to participate and will inform the instructor of any relevant limitations. In the event of an emergency involving a minor when a parent/legal guardian cannot be reached, I authorize TAAL to contact emergency medical services and obtain reasonable emergency care, with any resulting expenses being my responsibility.
Photo & Video Release: I understand that photographs and/or video may be taken during the workshop. I authorize TAAL School of Dance & the Indian Arts to photograph and record me or my minor child and to use such images or recordings for TAAL's website, social media, promotional materials, advertising, and other legitimate business and promotional purposes without compensation.
If the participant is under 18, this waiver and release must be completed by the participant's parent or legal guardian.
