- Strictly one (1) application per household.
- You must upload a copy of a lease agreement or utility bill (not Phone Bill) displaying your name as proof of Head of Household status.
- Funds will be prepared for the banking details provided in this form.
- Applications received by Tuesday each week will be processed for disbursement on Friday.
