Assumption of Risk & Release of Liability
I understand that participation in the above-mentioned event or activity may involve physical activity and may include actions or tasks that could result in injury.
By signing below, I acknowledge and voluntarily accept the risks associated with participation by myself and/or my minor child. I release and hold harmless ETO Martial Arts & Fitness, its owners, officers, instructors, employees, and representatives from liability for injury or harm arising from participation in the event or activity, to the extent permitted by law.
In the event that emergency medical treatment is considered necessary, I authorize ETO Martial Arts & Fitness to seek appropriate emergency assistance for myself or my minor child. I understand and accept financial responsibility for any costs associated with such treatment.
Photo & Media Release
I give ETO Martial Arts & Fitness permission to photograph and/or record me or my minor child during the event or activity. I understand that these photographs and recordings may be used by ETO Martial Arts & Fitness for business and promotional purposes, including its website, social media, advertising, printed materials, and other marketing materials, without compensation.

By signing below, I confirm that I have read and understand this waiver and voluntarily agree to its terms on behalf of myself and/or my minor child.
