Dear Parents & Guardians,
We are excited to offer our Drop-In Music Class and Music & Movement Class. These classes are designed to provide a supportive, engaging, and enriching environment where children can explore music, movement, creativity, and social interaction.
The Music Class is facilitated by a board-certified music therapist and focuses on age-appropriate musical experiences and activities.
The Music & Movement Class is facilitated by a board-certified music therapist and physical therapist and may incorporate music, gross motor activities, therapeutic movement, balance, coordination, and other age-appropriate physical activities.
Please read the following information carefully before signing.
Parent/Guardian Supervision
A parent or legal guardian must remain present and actively supervise their child at all times during the class. The parent/guardian is responsible for their child's supervision, safety, and participation throughout the session.
Parents/guardians are expected to follow the instructions of the therapists and class staff and to notify the therapists of any relevant concerns, limitations, or circumstances that may affect their child's ability to safely participate.
Participation and Assumption of Risk
I give permission for my child to participate in the Music Class and/or Music & Movement Class offered by Beach Baby Pediatric Therapy, LLC ("BBPT").
I understand that activities may include singing, playing musical instruments, dancing, rhythmic activities, gross motor activities, balance and coordination activities, and other age-appropriate music and movement experiences. I understand that the Music & Movement Class may involve physical activity and therapist-guided movement.
I understand that participation in these activities involves inherent risks, including, but not limited to, falls, trips, collisions, muscle soreness, or other accidental injuries. I voluntarily assume the risks associated with my child's participation and agree to be responsible for supervising my child throughout the class.
I understand that these classes are educational and recreational/therapeutic enrichment activities and are not a substitute for individualized medical care, physical therapy, occupational therapy, or other healthcare services.
Equipment and Property
I agree to use BBPT's instruments, equipment, supplies, and facilities appropriately and to encourage my child to do the same. I agree not to intentionally damage or misuse any equipment, instruments, supplies, or property belonging to BBPT.
Release and Liability
I release and hold harmless Beach Baby Pediatric Therapy, LLC, and its owners, officers, employees, contractors, therapists, agents, representatives, and volunteers from claims, liabilities, damages, or expenses arising out of or relating to my child's participation in the Music Class or Music & Movement Class.
I understand that I am responsible for determining whether my child is physically and otherwise able to participate safely in the activities offered.
Photo, Video & Audio/Media Release
I understand that photographs, digital images, video recordings, and/or audio recordings may be taken during classes. I understand that these materials may include my child, myself, my voice, or surrounding activities and individuals.
I hereby grant BBPT permission to photograph, record, reproduce, and use these materials for the following purposes:
To communicate with me regarding my child's participation in the program;
Educational and professional purposes
BBPT's promotional and informational materials, including its website, social media, brochures, printed materials, presentations, videos, and other media.
I release BBPT and its owners, officers, employees, contractors, therapists, agents, representatives, and volunteers from claims arising from the authorized use of these photographs, recordings, or other media.
Revocation of Media Consent
I may revoke my media consent at any time by providing written notice to BBPT. I understand that revocation will apply prospectively and will not require BBPT to retrieve, destroy, or discontinue use of materials that were created, published, distributed, or otherwise used before BBPT received my written revocation.
