Application Process
The Baraboo Police Officers Association - Baraboo Community Heroes - is proud to host our annual Holiday with a Hero event — a heartwarming experience designed to bring joy and connection to local children this holiday season.

This special event is available to children in grades 1-5 who reside within the Baraboo School District, and whose families have financial difficulties and/or significant life difficulties to which the child would benefit from the opportunity to connect with one of our local heroes for the holiday season.

During this memorable experience, selected children will be paired with a first responder "hero" for a day of fun, friendship, and holiday cheer. Activities will include gift shopping, gift wrapping, crafts, snacks, and time spent getting to know the everyday heroes who serve our community. Children will also get the opportunity to meet with Santa Claus and Mrs. Claus!

Please note:

This is an application-based event. Completion of the application does not guarantee participation, as we anticipate a high number of responses and have a limited number of spots available. Applications are due by November 10th, 2026. Families will be notified if their child has been selected by November 13th, 2026.

I also understand that I will be required to acknowledge participation in the event should my child be selected. If my child is selected and I do not respond, I forfeit their spot. If my circumstances should change, I understand it is my responsibility to notify the Baraboo Community Heroes at contact@baraboocommunityheroes.org. Does your family meet the 2026 Federal Poverty Guidelines and the Lower Living Standard Income Level Guidelines? This answer does not automatically aid or disqualify a child from participating in the event. This is for data tracking purposes.
Please explain why your child would benefit from the Holiday with a Hero event. We will prioritize needs first, such as warm clothing, shoes, etc. Any leftover funds will then be applied towards wants, such as toys, games, etc.

Parent/Guardian Consent for Transportation
I hereby give permission for my child to be transported by authorized personnel from their school to the Holiday with a Hero event at Baraboo Arts Center, and subsequently from Baraboo Arts Center to Walmart and back as part of the scheduled activities.

I understand that all transportation will be supervised by the Baraboo Police Officers Association - Baraboo Community Heroes - and participating heroes, and every effort will be made to ensure the safety and well-being of your child throughout the event.

Liability Waiver
I, the undersigned, am the parent or legal guardian of the minor child listed below and hereby give my permission for their participation in the Holiday with a Hero event, organized by the Baraboo Police Officers Association - Baraboo Community Heroes.

I understand that participation in this event may include activities and transportation that involve inherent risks, including but not limited to: travel by vehicle, walking in public areas, interactions with others, and participation in supervised activities. I voluntarily assume all such risks on behalf of my child.

To the fullest extent permitted by law, I hereby waive, release, and discharge the Baraboo Police Officers Association - Baraboo Community Heroes, its volunteers, partners, sponsors, first responders, employees, and affiliates from any and all claims, liabilities, or damages for personal injury, illness, property damage, or other loss that may occur during or as a result of my child's participation in this event.

Emergency Medical Consent:

In the event of a medical emergency, I authorize the Baraboo Police Officers Association - Baraboo Community Heroes - and event personnel to seek and secure emergency medical care, including transportation by ambulance, evaluation, and treatment by licensed medical professionals, for my child. I understand that all reasonable efforts will be made to contact me first, but in the event I cannot be reached or the incident is an immediate threat to my child's life, I consent to emergency treatment as deemed necessary for the health and safety of my child.

I understand that I am responsible for any medical expenses incurred as a result of emergency care provided to my child.

Photo Release Waiver

By signing this release, I hereby grant the Baraboo Police Officers Association - Baraboo Community Heroes, its representatives, agents, and affiliates, the right to take photographs and videos of my child during the Holiday with a Hero event, and to use these images or recordings for promotional, educational, and marketing purposes related to the event, including, but not limited to:
- Social media platforms
- Event flyers, posters, and brochures
- News outlets and press releases
- Website and online content
- Promotional materials
I understand that these photos or videos may be used without compensation and that the images may be edited, altered, or used in any manner at the discretion of the Baraboo Police Officers Association - Baraboo Community Heroes.

I further agree that the Baraboo Police Officers Association - Baraboo Community Heroes - is not liable for any claims, demands, or causes of action arising from the use of my child's image or likeness.
