Dear OCSM Student,

You may use this form to report your completed community service hours and receive an official verification letter. Please note:

1. Your chapter's Community Service Coordinator must first log the service event in the official OCCM Community Service Event Log form. OCSM relies on the completion of that form to verify and approve the hours that you enter in the form below.

2. Please ensure you properly document the name of the organization, the date/time, description, and all the requested information with proper punctuation and grammar. The text you enter in this form will be automatically printed on the letter.

You will receive your approved verification letter within seven (7) days of your submission.
