CONFIRMATION AND SIGNATURES

By signing, I, as an adult over 18 years of age, or as the Parent/Legal Guardian of applicant, hereby agree and grant permission for The Spaatz Association to use comments, quotes, or photographs of me, my image, or my likeness, as a grant recipient, for any lawful purpose, including brochures, illustrations, marketing, or for web content to promote the Association and its services.

I understsand that funds are distributed on a reimbursement basis. If awarded a Spaatz Association Solo Flight I agree to provide the Association with receipts for all expenditures by uploading them to this form (https://form.jotform.com/262735683305057) . If I change my flight education plan, I will contact The Spaatz Association immediately at ScholarshipsAndGrants@Spaatz.org for determination if unspent funds are to be returned or if I may use them for my new plan.

I acknowledge all expenses must be made within 1 year of notification of award via email and that I am not eligible for reimbursements that occur after that date.

If I am selected for another scholarship that covers the cost of solo flight hours, medical exam, or ground school, I understand that I will no longer be eligible for expense reimbursement. Once I solo, no further flight time is reimbursable.

Failure to comply with these requirements will make me ineligible for future Spaatz Association grants and scholarships.

CONTENT, LICENSE, AND RELEASE

I, the undersigned, give to The Spaatz Association, Inc. (the “Association”), and to anyone authorized by the Association, my irrevocable and unconditional consent to use and reproduce a photograph containing an image of myself for use in any media or medium, including the Association’s Web site, for the purpose of promotion, marketing and publicity in connection with the Association or the Association’s grant program. Such photograph may be used and reproduced by the Association at any time and in any manner without compensation to me, and I waive any right to inspect or approve any such use. A description of the photograph(s) is as follows: All photos, audio, video, articles and other media related to the event that I provide.

I also agree that the Association may use my name and biographical information to identify me as the subject in the photograph in connection with the uses specified above.

I also agree that if I hold any copyright in the photograph, the Association is hereby granted a perpetual license in the copyright to the photograph in connection with the uses specified above.
