Appointment Deposit Form
ALL DEPOSITS ARE NON-REFUNDABLE

A $100 deposit is required for all appointments. Your appointment is not secured until a deposit is received!

Your deposit is deducted from the final price of your tattoo.

We require a minimum of 48 hours to reschedule or cancel your appointment

Your deposit is forfeited if:

• Reschedule or cancel your appointment without 48 hour notice
• You no call/no show
• You cancel and don’t reschedule within 6 months

Deposits can be made in person with cash or through whichever digital pay app your artists accepts. Please reach out directly to your artist for payment after submitting this form or your appointment date will not be held.
