Join us at One Rotary Center in Evanston (1560 Sherman Avenue) to raise awareness, and hopefully some funds, for polio eradication!

Rotary has contributed more than $3 billion and countless volunteer hours to end polio forever.  We and our partners have reduced the number of cases by 99.9%.  You may not hear about polio, but we're still working to eradicate the disease in two last countries.  Until it's gone, we're not done.  

Come to enjoy the festivities - with music and purple frosted donuts! - starting at 9AM.  You'll hear a bit about the campaign to end polio and the logistics for the walk.  And at 10AM sharp, we'll join thousands across eleven states and two countries to take the next step in ending the threat of polio.

In consideration of you accepting this registration, I, the participant, and any other participants (including minors) on whose behalf I am registering, intending to be legally bound do hereby waive and forever release any and all rights and claims for damages or injuries that I may have against the Rotary Club of Evanston (and any other participating Rotary/Rotaract clubs), Rotary District 6440, Rotary International, The Rotary Foundation of Rotary International, Cushman and Wakefield, U.S., Inc., the City of Evanston, and all of their agents assisting with the event, sponsors and their representatives, volunteers and employees (“Releasees”) for any and all injuries to me or my personal property. This release includes all injuries and/or damages suffered by me before, during or after the event. I recognize, intend and understand that this release is binding on my heirs, executors, administrators, or assignees.

I know that participating in a walk is a potentially hazardous activity. I should not enter and participate unless I am medically able to do so. I assume all risks associated with participation in this event including, but not limited to: falls, contact with other participants, the effects of weather, traffic, and sidewalk/course conditions, and waive any and all claims which I might have based on any of those and other risks typically found when participating in such physical activity. I acknowledge all such risks are known and understood by me. I certify as a material condition to my participation in this walk that I am physically fit for the completion of this event.

In the event of an illness, injury or medical emergency arising during the event I hereby authorize and give my consent to the event organizers to secure from any accredited hospital, clinic, physician and/or any medically-trained professional any treatment deemed necessary for my immediate care. I agree that I will be fully responsible for payment of all medical services and treatment rendered to me including but not limited to medical transport, medications, treatment and hospitalization.

Further, I grant permission to all the foregoing to use my name, voice and images of myself and any other participants (including minors) on whose behalf I am registering, in any photographs, motion pictures, results, publications or any other print, videographic or electronic recording of this event for legitimate purposes.

We reserve the right to postpone or cancel the event due to circumstances beyond our control such as a natural disaster or emergency or as required to protect the safety of participants and staff. We reserve the right to change the details of the event without prior notice. 

By submitting this registration, I acknowledge having read and agreed to the above release and waiver, and that I am authorized on behalf of any other participants (including minors) for whom I am registering to agree to the above release and waiver.. I understand that I am releasing the Releasees from liability.  I waive any right that I and any other participants (including minors) on whose behalf I am registering, may have to bring a legal action or claim of any kind relating to the event against the Releasees.
