Thank you for taking part. This survey asks for your view of the culture and health of Living Hope.
Living Hope Community Church Ltd is the data controller for the information you give. Please read the following before you continue.
• Taking part is voluntary.
• Your responses are confidential. They are read in full by two people outside Living Hope, who are named below. Inside Living Hope, no elder, staff member or director sees what you have written, with one exception: the data protection officer administers the survey account and can see the responses in it. She takes no part in the review and does not report what any individual has said. Living Hope must also answer a request from a person to see information held about them.
• The survey is hosted on Jotform, on servers in the European Union, in Germany.
• Your completed responses go to Dele Bamgboye, who is outside Living Hope and based in England. He removes names and identifying details. He then passes the responses, with those details removed, to Ashley Gabriel, also outside Living Hope and based in England, who produces the findings. Nobody else receives them.
• The findings are reported to Living Hope’s leaders and to Four12. The report contains no personal data, and no group of fewer than ten people is reported separately.
• Your written comments may be reviewed and summarised using secure artificial intelligence tools to help with that analysis — by each of the two people named above, on the platforms recorded at [platform to be named] and [platform to be named], and by Jotform’s own tools within the survey platform.
• Your response carries a code unique to you, so it can be identified. You can ask for it to be removed by quoting your code to Dele Bamgboye, at [his address to be set], up to the point the responses are passed on with identifying details removed, which is shortly after the survey closes on 16 November. After that the findings have been drawn from it and it cannot be taken back out.
• Questions about how your information is handled: Lorraine Birney, Living Hope’s data protection officer, dataprotection@livinghope.im. You may also raise a concern with the Isle of Man Information Commissioner, with whom Living Hope is registered as R683102.
