Fusion Force Liability & Release
Liability
I understand that;
A) You agree that by participating in physical exercise or training activities, you do so entirely at your own risk.
B) You agree that you are voluntarily participating in these activities and use of these facilities and premises and assume all risks of injury, illness, or death.
C) You expressly agree to release and discharge the instructor from any and all claims or causes of action and you agree to voluntarily give up or waive any right that you may otherwise have to bring a legal action against the trainer or instructor for personal injury or property damage. To the extent that statute or case law does not prohibit releases for negligence, this release is also for negligence.
D) The participant acknowledges that the participant does not have any physical limitations, medical ailments, physical or mental disabilities that would limit or prevent the participant from participating in the above-mentioned activity. If required, the participant will obtain a medical examination and clearance.
E) Liability waiver extends to any virtual training including live or pre-recorded content.
F) Fusion Force Studio Ltd. is not responsible for any loss of your personal property.
Photo Release
I understand that;
A) Fusion Force Studio Ltd. reserves the right to use photographs and videos taken during classes, workshops, performances and other affiliated events for the purpose of instruction, advertising, and promoting Fusion Force Studio Lt.d and its programs.
B) Students or parents of students who do not wish to comply with this policy must give written notification to Fusion Force Studio Ltd. prior to participation.
General
I understand that;
A) I agree not to reproduce, sell, share or distribute any recordings of Fusion Force Studio Ltd. virtual training program.
B) I understand all music used in training classes and performances is not property of Fusion Force Studio Ltd.
I understand that the participant would not be permitted to participate in the activity unless I signed this agreement.
Electronic Signature Agreement. By selecting the "I Accept" checkbox, I am signing this agreement electronically. I agree this electronic signature is the legal equivalent of my manual signature on this agreement.
I have had sufficient opportunity to read this entire document. I have read and understood it, and I agree to be bound by its terms.
