Leilani's Hope is committed to supporting individuals and families throughout Waterloo Region by connecting them with services, resources, and opportunities that promote stability, healing, and hope. We believe that meaningful change happens when organizations work together. Through collaboration, we can expand access to services and maximize the support available to those we serve. We value partners who demonstrate professionalism, integrity, compassion, and a passion to client-centered care.

When Leilani's Hope identifies a need that aligns with your services, we may refer a client directly to your organization or engage your services on behalf of a client.

Partners are expected to:

• Provide services in a professional and ethical manner.
• Communicate clearly regarding availability, scheduling, and service delivery.
• Maintain client confidentiality and privacy in accordance with applicable laws and professional standards.
• Provide invoices and supporting documentation as required.
• Notify Leilani's Hope of any concerns that may affect service delivery.
• Work collaboratively with Leilani's Hope to support positive client outcomes.

Leilani's Hope will only share information necessary to facilitate services and support for the client. Information will be shared on a need-to-know basis and, where appropriate, with the client's knowledge and consent.

As part of our partnership model, Leilani's Hope requires that service providers contribute an agreed-upon percentage of their fees received through Leilani's Hope-funded referrals. These contributions are reinvested directly into our programs and services, allowing us to:

 

• Support current and new individuals and families.
• Expand access to community resources.
• Strengthen the long-term sustainability of our charitable programs.
• Create greater impact within the communities we serve.|
  
  

The contribution percentage will be discussed and agreed upon before services begin and may vary depending on the nature of the partnership or service provided. The contribution amount will be e-transferred to the Treasurer at finance@lhope.ca following the payment for services rendered unless we agree to an alternate arrangement. Contributions submitted to Leilani's Hope will be considered eligible donations for income tax purposes with a charitable receipt issued by Leilani's Hope no later than February 28^th of the following year.

Leilani's Hope is committed to protecting the privacy and confidentiality of all clients, volunteers, donors, partners, and community members.
As a partner organization or service provider, you agree to:

• Treat all client information as strictly confidential.
• Use client information only for the purpose of providing agreed-upon services.
• Collect, store, transmit, and dispose of personal information in a secure and responsible manner.
• Limit access to client information to authorized personnel directly involved in service delivery.
• Obtain appropriate consent before sharing information with third parties, unless disclosure is required by law.
• Immediately notify Leilani's Hope of any privacy breach, unauthorized disclosure, or security concern involving client information.
• Comply with all applicable privacy legislation, professional standards, and ethical obligations.

Information shared by Leilani's Hope remains confidential and must not be used for marketing, solicitation, fundraising, research, or any purpose outside of the agreed-upon services without prior written consent.

The obligation to maintain confidentiality continues even after the partnership or service arrangement has terminated.

Partners are responsible for:

• Submitting invoices immediately following the date the service was rendered. Leilani's Hope will reimburse the full fee for the services rendered. In the case of a defined treatment plan where there are a multiple appointments planned (i.e. Dental or Vision Care), it may be typical to submit the invoice upon completion of the final treatment rendered. Your invoicing method should coincide with your normal practice.
• Providing accurate records of services rendered.
• Maintaining transparent and accurate financial records related to services provided through Leilani's Hope referrals.

Payments will be processed via e-Transfer by Leilani's Hope to the service provider's preferred email address as noted below or on the invoice. We do not subscribe to payment by cheque, debit or credit card to keep our expenses as low as possible.

Leilani's Hope is committed to fostering an environment that is welcoming, inclusive, respectful, and free from discrimination.

We expect all partners to:

• Treat clients, volunteers, and community members with dignity and respect.
• Provide services without discrimination based on age, race, ethnicity, gender identity, disability, religion, family status, sexual orientation, socioeconomic status, or any other protected characteristic.
• Maintain professional boundaries and ethical conduct at all times.
• Support a safe and positive experience for every client served through our programs.

By participating as a partner with Leilani's Hope, you acknowledge and agree to:

• Support the mission, values, and objectives of Leilani's Hope.
• Deliver services professionally, ethically, and respectfully.
• Maintain all confidentiality and privacy requirements outlined in this agreement.
• Participate in the agreed-upon Community Reinvestment Contribution model.
• Work collaboratively with Leilani's Hope to achieve positive outcomes for clients and the broader community.
• Uphold standards that reflect the integrity and reputation of both your organization and Leilani's Hope.

Leilani's Hope values the relationships we build with our Partner Practitioners and aims to maintain collaborations that support the wellbeing of our clients and community. However, Leilani's Hope may, at its discretion, conclude the partnership with the Practitioner at any time. Reasons for discontinuation may include, but are not limited to, client feedback, changes in organizational needs, or a determination that the partnership is no longer the right fit.

If Leilani's Hope decides to end the partnership, written notice will be provided to the Practitioner. When appropriate and when client safety is not at risk, a reasonable transition period may be offered to allow for the respectful conclusion of any active responsibilities. Leilani's Hope is not required to disclose specific client details or internal decision-making processes related to the termination.

Upon termination, the Practitioner agrees to discontinue representing themselves as affiliated with Leilani's Hope and to return or securely destroy any confidential or sensitive information belonging to the organization.

By signing below, the Practitioner confirms that they have read, understood, and agree to the terms of this Agreement, including the Termination of Partnership clause and affirms their commitment to a respectful, professional, and client-centred partnership.
